COME HAVE FUN BEFORE SCHOOL STARTS!

Join us on January 2nd, 3rd, & 4th and January 7th!

Camp Rodeph Sholom is a fun program with games, sports, cooking, art & more. Register your child for any or all days of camp. Regular camp hours are 9am - 3pm. Before care 7:30am - 9am and After care 3pm - 6pm are available for an additional fee. Lunches provided for $4.50/day or bring own dairy/veg lunch.

INFORMATION:
  • CAMP SESSION IS WEEKDAYS FROM WEDNESDAY, JANUARY 2nd -  FRIDAY, JANUARY 4TH AND MONDAY, JANUARY 7TH.  YOU MAY SIGN UP FOR ANY OR ALL DAYS OF CAMP.
  • CAMPERS SHOULD BRING A CHANGE OF CLOTHING FOR MESSY ACTIVITIES.  DAILY SCHEDULE WILL BE SENT OUT BY DECEMBER 23RD.
  • ANY MEDICATIONS SHOULD BE LABELED AND THE WRITTEN INSTRUCTION SHEET MUST BE COMPLETED.
  • KOSHER LUNCHES MAY BE PURCHASED EACH DAY OR YOU MAY PACK A DAIRY OR VEGETARIAN LUNCH.  LUNCH ORDERS LISTED BELOW ARE $4.50 PER DAY.  SNACKS WILL BE PROVIDED DAILY.
   Extended Care AM (730-900AM) $3/Day
   Extended Care PM (300-600PM) $8/Day

LIST TWO CONTACTS IN CASE OF AN EMERGENCY
PHOTOCOPY OF HEALTH INSURANCE CARD NEEDED
to participate in the programs at Congregation Rodeph Sholom including off campus trips.  In the event of an emergency, surgical or otherwise, and I cannot be reached, I hereby give permission for my child to be transported to the nearest medical facility and specifically authorize the representative of Congregation Rodeph Sholom to select a physician and/or authorize medical treatment, including hospitalization, anesthesia, injection, surgery or other measures which he/she feels are in the best interest of my child.  Congregation Rodeph Sholom is hereby released and held harmless from any claim, judgment, awards, settlements, or damages to any person or property arising directly or indirectly out of my child's participation in the programs at Rodeph Sholom, including off campus trips or the Congregation's selection of physician, hospital, or any other medical service for my child in a medical emergency, or in connection with the rendering of any such medical treatment.
